The sixth and final season of Nip/Tuck premiered on October 14, 2009 and concluded on March 3, 2010. Though a seventh season was originally set to air in 2011, the remaining nine episodes were merged into the sixth season.[1][2][3]

Christian and Liz engage in bitter divorce proceedings following the revelation that Christian is not suffering from terminal cancer, with a vengeful Liz wanting to take half of everything Christian owns. Matt reveals to Sean and Christian that he is working as a mime, but it is not long before his newfound talents lead to criminality. Kimber, after learning how to perform electrolysis therapy via night classes, appeals to become McNamara/Troy's on-site esthetician. Faced with the economic downturn, Sean and Christian turn to one-time rival plastic surgeon Mike Hamoui (guest star Mario Lopez), who strikes a deal with the doctors in which he pays 10% of his own money to use their surgical suites and help drum up business for the struggling practice, while simultaneously bringing in a new form of client: women seeking vaginal rejuvenation surgery. Despite overdosing, Sean's addiction to sleeping pills continues, driving away Teddy (Rose McGowan) who ends their engagement. Struggling to sleep, Sean hits the town and encounters a self-harming junkie (guest star Amy Price-Francis) who gets off on the injuries of others. Still desperate for money, Christian turns to an equally cash-strapped Kimber with a plan to create a range of Kimber Henry-endorsed sex toys molded out of his penis. Liz continues to try and take Christian for everything he's got, and later confronts Kimber over her escalating attraction to Christian. Two concerned parents (guest stars Joan McMurtrey and Todd Waring) request surgery on their disturbed son, who has had Satanic words and images tattooed and scarred onto his head. Mike (Mario Lopez) asks Christian for advice in sexually satisfying Kimber, but Christian's plans at ending their union have unexpected results. An Austrian socialite, Briggitte Reinholt, comes to McNamara/Troy for her bi-yearly plastic surgeries, and requests that leeches are used on her face to speed up the recovery process. Teddy finds her latest identity compromised when Briggitte recognizes her from her time in Las Vegas under the guise of "Dixie". A heterosexual transvestite (guest star Scott Atkinson) requests McNamara/Troy's help in creating a special mask that he can wear in public to hide his true identity.

Christian tries to help a woman with extremely large natural breasts, but her anger management problems create trouble.

A gifted pianist wants her parasitic twin, named Moira, removed from her shoulder in time for her upcoming concert debut. But removal of "Moira" creates some deep separation anxieties.

Shortly after her arrival, Erica announces her plans to take both Annie (Kelsey Batelaan) and Conor into her sole custody, manipulating an incarcerated Matt to testify against his parents in the process. Mike (Mario Lopez) announces his engagement to Kimber, an event which forces both Christian and Kimber to reconsider their own complicated relationship. Liz attempts to fix the situation between her and Christian. Christian sleeps with a woman he picks up in a bar, who soon reveals that she is a male-to-female transsexual who wishes to undergo surgery to become a man once again.

A psychotic prison inmate (guest star Patrick Kilpatrick) requests breasts implants for Matt, his cellmate, forcing Christian to make a potentially life-threatening decision on whether to operate on his son. Julia plots revenge on her mother after years of emotional abuse. Alexis, now living as Alex, realizes he is only attracted to straight men and requests breast surgery.

Sean is surprised by the sudden re-appearance of his long-lost brother Brendan (guest star Neil Hopkins), a drug addict and con artist who was believed to be dead. Brendan asks for surgery to repair his meth-ravaged face in order to start up his own business, but Christian doesn't trust his proclamations to be clean. A Swedish banker (guest star Hans Tester) requests surgery on his adopted son's face to make him look more like him. Christian forms an immediate friendship with the boy, but quickly discovers a shocking truth about the boy's relationship with his father. Christian faces financial ruin after he is left hundreds of thousands of dollars in debt.

Sean suffers an ethical and moral dilemma when he is offered a deal which, in return for performing liposuction on an obese prisoner about to be executed, would result in Matt being released from prison. Christian, after discovering that Kimber is pregnant, orders her to have an abortion or lose him in the process. His ultimatum grows when he promises to marry her if she goes through with what he wants. A desperate Kimber turns to both Liz and Mike (Mario Lopez) when deciding how far she is prepared to go for the man she loves. Matt discovers information that could jeopardize his possible release.

A new patient, suffering from the self-mutilating disorder Lesch-Nyhan syndrome, comes to McNamara/Troy for reconstructive surgery. Guest starring George Newbern.

At the same time, Julia arrives in Los Angeles with surprising news about her personal life, a revelation which provokes both Sean and Christian to make separate advances on her. Liz is told of the health of her unborn child. A married pair of elderly Holocaust survivors come to McNamara/Troy for surgical removal of their individual numbered tattoos, inflicted on them by the Nazis. But the removal provokes deep guilt in one of the pair. Guest starring Melonie Diaz.

Both Julia's worries and a vision of Kimber force Christian to reconsider his long-standing relationship with Sean. An elderly Japanese porn star comes to McNamara/Troy requesting surgery to remove surgical scars before his big debut in an American porn movie. Guest starring Melonie Diaz.
