Introduction: Throughout PATCH, data are used to guide the process. Phase II begins when the community group forms working groups to assess community health status and needs. The community group uses data as it selects health priorities in phases II and III. In phases III and IV, the community group may want to obtain additional information on selected health priorities, target groups, and related community resources as it designs appropriate and effective interventions. In phase V and throughout the process, data are collected and examined to evaluate interventions and identify areas for program improvement.
